Passport and Travel Documents

The following documentation must be submitted when applying for New Passport, Stolen/Lost Passport, Renewal and Extension of Passport or Emergency Travel Document:

New Passport:

  • Old passport attached with photocopy of pages 1-5, also last page with (DN) number.
  • Five (5) photos standard passport size with light blue background.
  • Photocopy of any U.s. Identification Card (1.D.) i.e., green card, work permit, driving license, state 1.D., college 1.D., etc.
  • Marriage certificate for married women.
  • Birth certificate for those who has.
  • Proof of any change of name by Deed Poll.
  • Finger prints must be taken at the Tanzania Embassy only.
  • Fee is $50.
  • Self-addressed returned envelope by FedEx or any other secured mail: Fee $20.

Stolen/Lost Passport:

  • Police incident report.
  • News advertisement.
  • Notarized information on the passport loss.
  • Letter stating reasons to apply for new passport.
  • Proof of being in U.s.A., green card, driving license, work permit, or any other 1.D.
  • Birth certificate (copy)
  • Five (5) passport photos with light blue background.
  • Finger prints must be taken at the Tanzania Embassy only.
  • Immigration file number (DN)
  • Fee is $100.
  • Self-addressed returned envelope by FedEx or any other secured returned envelope: Fee $20.

Emergency Travel Document:

  • Three (3) passport size photos.
  • Complete the appropriate application form ( Click here for forms )
  • Old passport, together with photocopy of page 1-5 and last page with (DN) number.
  • Letter explaining reasons for applying for travel documentation.
  • Photocopy of any U.s.A. LD. card or documentation.
  • Self-addressed return envelope or a payment of $20 will be charged.
  • If your old passport was stolen/lost, all procedures for stolen/lost passport are to be followed before lodging an application.
  • You may be required for a telephonic interview, as the Consular Officer may determine.
  • Fee is $21.

Extension or Renewal of Old Passport:

  • Three (3) passport size photos.
  • Letter explaining why you need your passport to be renewed.
  • Old passport.
  • Self-addressed return envelope or a payment of $20 will be charged.
  • Complete the appropriate application form ( Click here for forms )
  • Fee is $35.
  • U.S.A. Identification Card (ID)
